Incident Reporting
All accidents, incidents and near misses* must be recorded on the online form below within 24 hours of the incident occurring, ideally by the person dealing with the incident. This information is reviewed by the Office for Safeguarding Children and may be shared with the National Spiritual Assembly. (* A near miss is defined as an event or situation that could have resulted in a serious outcome, but was avoided by chance or by intervention)
